At your initial appointment, we will take the required time to properly diagnose your immediate dental concerns, review your past medical and dental history, and create a treatment plan that provides for your optimal dental health. With your cooperation, we will complete a thorough comprehensive dental evaluation.
Dr Bradly Baus will discuss with you all of your treatment options. Often, treatment can be done or started the same day as the consultation. However, patients with a complex medical history and/or complex treatment plan may require additional appointments.

- X-RAYS Please request your most recent dental X-rays be emailed to our office.
- MEDICATIONS A list of medications you are presently taking.
- DENTAL INSURANCE The front and back of your insurance card.
- NEW PATIENT FORMS Completed new patient forms .
Please alert the office if you have a medical condition that may be of concern prior to treatment (i.e., diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.) or if you are currently taking any medication (i.e., heart medications, aspirin, anticoagulant therapy, etc.) or if you need additional assistance (i.e., cane, walker, wheelchair, bathroom, etc.) Your safety and comfort is our priority.
IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian.
